Change of Address Form

Please find attached the Change of Address Form. This needs to be filed out and signed for the changes to take place.
Once the form has been filled in and signed, it can be scanned and emailed back to council@armidale.nsw.gov.au or alternatively it can be dropped into the Customer Service Centre.
